Sierra Forever Families is a private non-profit foster family/adoption agency that focuses on finding permanency for children and youth in the foster care system. We are seeking a creative, enthusiastic and passionate social work professional for a full-time position as a Youth Permanency Worker (YPW) in our Destination Family program in our Sacramento office.

Position Summary
The Youth Permanency Social Worker (YPW) is responsible to utilize mental health, social work best practice and innovative techniques in finding permanency for youth in foster care. The chosen candidate will be responsible for EPSDT Medi-Cal billing, assessments, treatment plans, CANS and other documentation requirements. The position will require case management skills in the coordination of services with the other members of the Destination Family Team. The YPW is part of an interdisciplinary team that includes a clinician, Parent Partner and Youth Skills Trainer.

Sierra Forever Families is a private non-profit foster family/adoption agency that focuses on sustaining permanency for children and youth in the foster care system and provides an array of other community-based services to families and children. We are seeking a creative, enthusiastic and passionate licensed therapist for a full-time position as a Clinician/Program Lead supporting our Placer County Permanency Support Services.
The Clinician—Program Lead will manage all aspects of the start up and day-to-day operations of the program and ensures regulatory compliance at all levels. Provide consultation and support to the rest of the team which will include a clinician, case manager, parent partner, and administrative support. The team will provide community based individual, group and family therapy to clients referred to the program. Responsibilities include participation in the development and monitoring of treatment plans and EPSDT Medi-cal billing documentation. This position will carry cases and bill for services.
